The main aim within our services is to provide personal care and create opportunities that are conducive for ongoing rehabilitation and recovery whilst making a positive difference to the lives of our service users by building essential community links and strategies that support re-integration into the wider community through working in partnership with everyone.

Our model of care is based on an integrated approach whereby family, service users, commissioners and carers are all involved in the rehabilitation and recovery of the individual. Our mission is to work in partnership to achieve the best outcomes for the people we support. We work collaboratively within the wider system to provide services that are creative, innovative, and sustainable.

The service user will receive support within their own independent tenancy. The support we provide is aimed at increasing independence, and a more outward-looking community-based programme for those to enhance full social reintegration and practicalities of day to day living. We provide personal care to people as part of the support that they need to live in their own homes. We provide personal care under separate contractual arrangements to those of the person’s housing.

We support individuals to become valuable, engaged members of their communities so that they can lead more meaningful, fulfilling lives.
